Vestibular migraine, while it can sometimes involve tinnitus or ear fullness, does not typically cause fluctuating hearing loss and is primarily defined by its association with migraine features (headache, light/sound sensitivity, aura). The underlying pathology also differs: Meniere's involves a build-up of fluid in the inner ear (endolymphatic hydrops), whereas vestibular migraine is a central nervous system disorder.
